I'm trying to find the best practices for products which are in subcategories.
If I have "product A" which is in "Cat-A > Sub-A" should it be added to both "Cat-A and Sub-A" or just "Sub-A"? "Cat-A" does have lots of text but no products assign as they are all in "Sub-A".
Should all product be added at every level and allow a filter to do work?
I hope I've explained this well enough, I have tried searching google for an answer but couldn't find anything.
No OC version posted. However, your question is more about logic and common sense rather than technical. Take note that the default installation of Opencart has three restricted levels of categories with sub-categories. If you wish to use unlimited depth with sub-categories, I would suggest to use an extension from the marketplace.

I think its best to add your product under the right subcategory which it belongs to.
Category page should feature all the sub-categories that comes under it.
Also, instead of just putting all your products under a single subcategory, you should create multiple categories and subcategories based on your products. There will be a logical division for all your products which can make the navigation easier for the customers.
An easy and intuitive navigation structure is very essential for ecommerce website, Putting all your products under one place is definitely not the best practice. That being said, you should never add products under a category page. Category page should feature all the subcategories that should come under it.
