Product option with Form

hi, how i can add meaurement with product using form on opencart.. I am trying to do that .. but i am not getting the idea how i can do that ....

they have add product options and then if u choose customization .. it will redirect to measurement page and once you submit it.. the measurement will added with that product... you can save measurement also.

please help me how i can achieve this functionality on my website..
