Form re-direct

I have been looking for a way to do the following:

Have a simple text box labeled "enter your code" with a submit button in the header or footer of my main site.

When the customer enters a code, lets say 12345, they get re-directed to (12345.mysite.com)

They enter 98765 they go to 98765.mysite.com, etc...
