Upload Your Own Art extension failed to activate

I have purchased the "Upload You Own Art" extension. When I try to click "Upload Your Art" I get this message "Notice : Undefined index : md5hash in /webroot/dezigner/public_html/catalog/controller/studio/upload_art.php on line 0 # Show Invalid Message".
I have tried uninstall and reinstall both "Opentshirt" and "Upload Your Own Art" but the problem persist.
Can anyone help ?
