Product Warning Messages - any extensions do this?

searched for "warning" in the extensions area but didn't quite find what we need, the following was the closest:

we're looking for something that will allow us to display a custom warning message for specific products

each message could be specific to that product

also, some warning messages could require a checkbox to be checked before the item could be added to the shopping cart, for example:

Item 123
(styled large, green, in box) WARNING - EATING THIS PRODUCT COULD TURN YOU INTO A FROG
Please have Adult supervision
[checkbox to agree and to enable purchase]

Item 435644
(styled large, red, box with dash border) ATTENTION: FRAGILE
Please handle carefully
[no checkbox]

Can you arrange for FTP/Admin access credentials?

i was kinda hoping that someone would want to develop this and offer it in the extension marketplace for like $15-20
