I have created a coupon which gives a fixed discount on a specific item. The dates have been set properly, and the coupon works as expected.

I also have the following settings, which I believe mean, the coupon can only be applied to a discount on 1 item, and only used once by each person who uses it.

Uses per coupon: 1
Uses per customer: 1

The problem is that I can use the coupon a second time using the same account. The coupon use is showing up in the history tab, and if I open the invoice as the customer, I can see that the coupon code was applied and a discount given.

What else should I check?

I have the same problem. Number of coupons per customer is not limited.

Out of curiousity, what payment method are you using ? if I use SagePay standard it's fine, but if I use SagePay iFrame it doesn't work... also works with CoD. So I'd guess it's based around a status that is being returned by the payment module or something like that.
