Icon Not Showing

I am having an issue with the icon showing up. I have created a new icon which is 16x16 and .png but it does not show. The default cart image does not show either only a square with dotted lines. The default shopping cart image has never display only the square. Any idea?

this may be annoying, but did you clear your cache?
firefox, chrome, try crtl+shift+f

Could be that 666 in your username is beckoning the beast to come play mischief on your server.

i got the same problem , icon show 500 internal error

Might be a daemon war going on inside the host.
