Urgent help please. Can't access opencart admin

Opencart 1.5.5.1 (not new install)
Custom theme.

We recently installed a extension (http://codecanyon.net/item/uiform-unive ... er/6648154) Followed instructions as carefully as possible, successfully installed the form and it works under the subdirectory of our opencart store, however for some reason the opencart admin is no longer working and we cannot access the back end of opencart.

After pressing the Login button we get the following error:

Could somebody really help?

You should contact the extension author to help. What you've provided isn't enough info for anyone to help. The code is behind a paywall (which means you shouldn't post it), so aside from the author, you'd need to hire someone from Commercial Support to help.

Thanks for the help, I've sorted it. When installing the extension, it was installed on our default opencart database. removed it and created a new database for the extension and imported from backup the opencart database.
