Gift Voucher Issue - Double Vouchers Being Sent

Hi all, I hope you can help me, this is issue is causing me a lot of problems!
Sometimes when customers buy gift vouchers the recipient is sent two gift vouchers instead of one.
Essentailly they are paying for one gift voucher and recieving two. This only happens some of the time and I am unable to figure out why. Is anyone else having this problem?
I have not been able to find any info about this issue online, so I am thinking it could be an extension conflict somewhere.
Can any one advise which files control the gift vouchers? I though I might try to replace them with the deafult files.
