First, flush /vqmod/mods.cache and /vqmod/vqcache/*.* files, plus /system/cache/*.* files. Retry. If that works, you're good to go; sort of. Otherwise, . . . check these anyway.

Probably or possibly hacked. Look in /download/ for anything weird, including *route* and *jpg* and *php* files, and ANY .php files in the size ranges of 29-34 kb or 74-78 kb. Look at your tree for 777 permissions, reset all dirs 755, files 644, see that they stay that way (if they change, then you have a problem). Look for any .dirname/ (dot) directories, usually with 777, if you have those, then you have a problem. This, "<b>Fatal error</b>", appears to be involved in attempting to shift filespecs to execute an embedded .php extension.

Compare timestamp of each such error to timestamps of http traffic (Apache traffic logs should be accessible in host control panel or from support, they are exceedingly specific, file by file, not just which village idiot space cadet landed for a while). If there is a correspondence, then there are several ways to ban the address range(s).
