Product Return module doesn't work

Product return module doesn't work at all. When I hit save after entering the transaction details, nothing happens. Anyone know how to fix this? I also see other unanswered questions in the forum about making returns work. Has anyone ever used the returns feature successfully? Anyone know how it can be fixed?

(And a side note - the return module leaves a lot to be desired. Once the order number is entered, it could auto-populate the fields, but instead makes you fill them in. There is no evidence that these fields are being used as a data entry check. Not useful at all. Autocomplete for the product ID doesn't work either.)
