Best photo gallery extension?

I've been looking for a photo gallery extension for some time now, and they're all insufficient in one way or another. Does anyone have a recommendation for the best extension which meets the following criteria?
Must have:
1. Categories, including at least one level of subcategories (i.e. minimum two levels deep).
2. Unlimited categories and images (or at least a good number like 50+ each).
3. Auto-generated image thumbnails.
Nice to have:
4. Auto-generated category thumbnails or the ability to upload/choose a category thumbnail.
5. Category list/thumbnails above or beside the image listing (e.g. similar to the Categories box when browsing for products in the store).
6. Not managed via System > Design > Banners. This is certainly not a deal breaker, but I find this to be a clunky method of maintaining lots of images in a photo gallery.
7. Bulk upload feature.
