I installed a clean OC 3.0.3.9 and installed the PayPal extension from the marketplace. It looks and works fine for me.

Well something happened, I'm not sure what, but something, because now it is working fine.
Yesterday I decided to take another look and see if the issue was good enough to maybe just change some stylesheet properties to help make the "Pay with Card" button easier to spot, and I pulled up the site on my phone and noticed the layout was perfect. So I tried again on my desktop browser and it was laid out perfect as well, in all different browsers.
I have no idea what happened, perhaps they did something with my account at paypal to verify the account or approve something (since I never got their verification emails), but it's working now. Everything is in the right spot and looks perfect.
