Contact us form isn't sending emails.

Hello!

I am running Opencart Version 2.3.0.2 and I have Basel theme installed in my store :

When trying to send a message in my contact form: https://www.adderah.com/contact

I get "Failed to load resource: the server responded with a status of 500 ()"
AND
"A bad HTTP response code (404) was received when fetching the script."

Could anyone tell me how to fix this issue? Although, I have my mail setting set correctly. Please check the attachments, any help would be appreciated!
