Vqmod and Costumer Order Product Upload

My problem is that it seems i am getting two instances of the extension printed on the Front End which also leaded to having some vqmod errors on the back end when i re uploaded the xml file for vqmod. i manage to remove the lines in the xml that where giving an error but i can't seem to figure out how to make it display properly as it is not displaying the upload properly on the product pages i think it is related to how it is printing double on the order/checkout section. If you look at the image below you'll see that the field on the product is repeated as well.
