Ajax Optional Fees

I just purchased the Ajax Optional Fee (Insurance, Giftwrap, Insurance, etc) from qphoria. Installed all the files in the extract to admin, catalog, docs and vqmod. Successfully installed and edit the Ajax optional fees in Extensions - Order Totals. Named the fee "Priority Mail", flat rate $2.50 and sort no. in between Subtotal and Total. Running a test order, the optional fee doesn't show in the shopping cart nor checkout. Help please.

It's unlikely anyone in the forums will be able to assist you. If you haven't already, you should contact the developer of the extension, and patiently wait for a response.
