Total based shipping ext. or coding

Can someone recommend an extension or perhaps share some coding for the following shipping requirements:

Flat rate of $8 shipping in specific geo zone for orders over $40.
Free shipping in specific geo zone for orders over $75
And $11 for orders under $40 .

Searched "all" day looking for something, ended up being more baffled than when I started.

Total-Based Shipping can do this. It can restrict rates by total and geo zone, so you can create both a flat rate and free shipping rate for the various order total amounts.
