How do I allow customers to choose insurance on USPS and have it calculated based on order total? Is there something I'm missing in setup? Is there an extension for this?

There's no option for this in the built-in USPS extension. If you want to have insurance be an optional fee they can select, you could use my Optional Fee/Discount extension to do that.
If you want to make it a mandatory fee, you could use the slightly simpler Total-Based Fee/Discount instead.
