[RELEASED]Shipping to Payment combos + Payments Fee

Checkout Manager is a very powerful module which allows to eliminate some very common and very annoying checkout "incompatibilities". For example with Checkout Manager you can "eliminate" incompatible Shipping and Payment methods, for example Pickup from Store + Cash on Delivery or Courrier Shipping + Payment at Store.

Also you can add a Fee at each Payment method, for example at Cash on Delivery.

Checkout Manager currently supports:

1) Shipping - Payment methods combinations: Foreach shipping method you can select the preferred payment method(s). Other Payment methods will not be visible.

2) Specific Payment method(s) selection when Gift Vouchers or Download products are purchased.

3) Fee amount addition at each payment method. It can be a fixed amount or a percentage amount at order total.

Admin can choose Specific Payment method(s) to be visible when Gift Vouchers or Download products are purchased. Other incopatible payment methods wont be visible.
