[vqmod] Fixed Price Gift Voucher with Image

Features:
+ Remove amount box and use voucher theme name to set price of gift card (i.e. "10 Gift Card" will set price to 10 automatically)
+ Voucher image is shown on Purchase Gift Certificate page and shopping cart
+ Support multiple currencies
+ vqmod - no change to core files
