1. Do users have to be logged in to submit tickets?

2. Can tickets be tied to order numbers if related to ticket contents?

3. Can staff create a ticket and send it out to user/customer?

4. Does this system retrieve email and convert it into a ticket, or does customer have to come back to site and enter their reply?

great extension.
if i may suggest:
1- change the edit next to the ticket in admincp to "view"
2- add "Tickets waiting reply" in admin dashboard below "Affiliates Awaiting Approval"
3- hook a client's orders in the form as a dropdown menu showing all his orders to choose from.

Thanks for the update. Just follow the instructions as per the previous version extension right?

I tried to "Install Support" but it says

Permission Denied!
You do not have permission to access this page, please refer to your system administrator.

System->User->User Groups->Edit -> All select checkbox and install try again
