how to add categories

when i try to add new opencart categories,i got this error.
i try to delete many times.categories has deleted.but there is this massage in up in homepage
my ocart version 1.5.1.3

The most generated errors being found on Opencart forum originates from contributed programming. The increased post counters are caused by redundancies of the same solutions that were already provided prior.
