Is it possible for me to leap straight from version 1.4 to 1.5.1.3.1?

What steps are you doing when trying ?

OK, in 1.5.x you first create your options in catalog - options

Basically you create all your possible sizes/colours etc in there

Then you go edit a product and on the option tab start to type an option name and voila it will appear.

The green plus icon does nothing other than to indicate that is where you add options (weird I know)

Once you have selected your option from the magically appearing dropdown, you then add each of the possible values for that product.
