Another thing thats increase our sales is a PayPal button on the shopping cart page so that the customers does not have to enter any personal details at all on the site and it takes them straight to paypal to complete the payment.
At the moment we are running a few carts on opencart and I think this is the only thing hindering sales.

For me I'd like to have not only discounts at a product level but also group discounts for customer groups at a site level, cart discounts for sales price (e.g. 5% off if you spend £50, etc).
Really, in an idealistic world there would be a fully functional discount system allowing you to set discounts on any variable at any level and in any number of combinations.
As an example, I take 10% off the RRP of all my products (where I receive a 10% bulk discount), this could be achieved by having an RRP field and a sales price field per product and output it's display in the customer expected Amazon style:
R.R.P: £29.99
Price: £26.99
You save: £3.00 (10%)
A percentage discount on the RRP wouldn't always give the price I actually want to sell at, so a flat price here with autocalculated figures for display would be better, however I'm willing to accept greater experience here.
Continuing the example, I want to give some local customers a free shipping option, so that I can deliver in person. Currently I am doing this with a discount code giving them free shipping. I would rather they get the option at checkout automatically based on their group as well as the possibility of a set bonus discount of 2.5% of their order.
On top of that I would like to offer a tiered percentage discount based on the value of the products being bought at checkout, 5% for £50, 7.5% for £100 and 10% for £200. With again the option for free shipping on a £200 sale.
Some of this I can do by fiddling with coupons, but I think I'm not the only one who would like to see at least some of these improvements being addressed with the discount system; hell you might even come up with a better way of doing it that would provide me with even more options to keep up with/beat my competition (who all seem to be able to do these things with their e-commerce sites I might add).
I should also add that I'm still using 1.4.9.6, I've had a look at and a play with 1.5.0 but it didn't offer me enough (at the time of looking at it) to warrant an upgrade. A revised discount system along with the already implemented reward points would however give me that incentive to upgrade.

The ability to change the heading of the module too.
Or a Paypal express button next to each product to checkout immediately with Paypal without even going to the cart page!
Obviously needs to be optional as it wont be viable in configurations where shipping and tax etc needs separate calculation.

No many people even good devs seem to think that, and have said as much on the forums, but they have obviously not tried it. You should be able to have a different set of images for each language of your site.
(Currently you have a single set of images which is displayed on every language you have installed. There's no point showing English adds for German speakers or vice versa.)
Same comes for currencies. If customer prefers paying in GBP, there's no point to show prices in Euro.
1) Cloud based server use for downloads (Amazon S3/CloudFront, Rackspace et-al)
2) Paypal's "Adaptive Payment" system (allows payments to be split between more than one receiver)
3) Shift or provide option for file downloads to be hosting at server root (instead of inside 'public' folder)
FYI.. Magento Cart has this option.
