Need Flat Rate Shipping for Canada and International

I am using v.1.4.9.1 and currently have 3 geo zones. US, Canada and World (all other countries). I have setup a flat rate shipping for US. How can I setup flat rate shipping for CA and then a separate flat rate for International. Also is there a way to do a flat rate and then charge certain amount for each additional item in the cart?

Basically use weight based shipping
Make 2 geo zones - canada and International

For international use the module
http://www.opencart.com/index.php?route ... order=DESC
to add all the countries, then just remove canada (cos you want to exclude canada from the international list)
