How can I add a custom form at the check out stage. I am setting up a site that sells wedding invitations and I want the buyer to fill out a form with all their wedding details.

unless you are familiar with php programing you would probably have to pay someone to custom code this for you.

Thanks for that I may do that.

Another way would be to attach a .pdf to a thank you email. Or is this equally as complex.

One option I used was to use a free mail script & made a custom form
the I taged it to a menu & use a iframe to call the form html page
One option you could do with this script is once a order has being recieved yuo could emal them back a confomation email with a link to the form along with a JOB or PO number or something that woudl be unique to each order. I am not a programmer if you have a look at the link below this might give yuo an idea

I've done this for a bespoke clothing company for entering customer measurements and storing them as profiles.
I added this to the product page as well in case the person wanted to enter their measurements directly.
You don't have to do it during the checkout process, but this is also possible.
