One option I used was to use a free mail script & made a custom form
the I taged it to a menu & use a iframe to call the form html page
One option you could do with this script is once a order has being recieved yuo could emal them back a confomation email with a link to the form along with a JOB or PO number or something that woudl be unique to each order. I am not a programmer if you have a look at the link below this might give yuo an idea

I've done this for a bespoke clothing company for entering customer measurements and storing them as profiles.
I added this to the product page as well in case the person wanted to enter their measurements directly.
You don't have to do it during the checkout process, but this is also possible.
