Add selection box to checkout page

Hi,

I want to add a time period select box in checkout page like customer can choose the time to delivery.

I refer this page http://karlmac.com/2011/07/adding-custo ... ing-vqmod/ so I can add a text box, but I don't know how to change to select box.
