Hi guys, can anyone PLEASE help me find an extension that will let me set my options like they do on the following product page:

I must have tried half a dozen demos including from some of you on here but I'm just not clear what your plugins are capable of. I need this extension today to if you could get back to me asap I'd really appreciate it so I can order it!!
