Updated to v300.1

Due to limitations in the opencart.com license system, a new extension was released on opencart.com for OpenCart 3.0 compatibility. However, free updates are still available for anyone that has purchased the extension in July 2016 or after --- just contact Clear Thinking here: http://www.getclearthinking.com/contact

Release Notes:
- NOTE: This version changes the filenames and internal code for the extension from "form_builder" to "form_builder_pro". This means it will show up as a new extension in Extensions > Modules. For directions on how to port your old forms to the new extension, make sure to read the instructions.txt file, or you can contact me at www.getclearthinking.com/contact

- Added: compatibility with OpenCart 3.0.0.x
- Added: native compatibility with OpenCart 2.3.0.x (You can remove the compatibility fix, or keep using it if you want to)
- Added: info in the admin page to see which layouts the module is assigned to
- Fixed: HTML support for the module heading setting
- Removed: compatibility with OpenCart 1.5.x and 2.0.0.0 for new versions
- Updated: success and error messages are now shown in an alert banner rather than an alert pop-up, to fix issues with HTML not working in those fields

- Confirmed: compatibility with OpenCart 3.0.2.x
- Fixed: File Upload fields were not working in v300.1
- Fixed: if using multiple shortcodes with whitespace in between them, the placeholder text for the field wouldn't appear if the shortcodes had no data
