Opencart Company detail Vqmod

I have developed a vQmod Company Detail. Which will work as Company ID with all its functionality except for affiliate. When I tested (OC Ver 1.5.4.1), It displays Company detail field in admin/sale/customer_group, but when I save the option the blank page displays. Can someone verify the code and rectify the problem please. Here is the vQmod XML File:

a blank page usually means a php error. If it happens when you saved, usually it is the code for inserting / editing the data into the database

I am not able to find out the problem. Can someone test this code on their own website?
