We have a customer that is wanting a checkout survey. Not expected for free, we are willing to pay for the development of this mod if the price is reasonable.
As part of the checkout process we would like the customer to be asked "How did you here about us?"
The following options should be in the list. The ability to edit this list from the cart would be preferable as well as maybe some other basic functions like changing it from required or not required.
Google
Yahoo
Bing
ETC..

It would be slightly better if it could be made to use the order total system. Then it would be a lot less to change during upgrades (mostly dynamic with a small 2 line edit of the tpl file). I have a charity mod that uses it like this for input and I think I could do something like that for this as well. I assume you would want this to appear on the admin order page?

I was just thinking about this last night.

If you're just asking for an extra field in the db as in "how did you find us?" is not that all difficult.
If you use the order check total, wouldn't it then show up every time a customer returns?
I tend to use a shop again once my first order goes through ...
I was thinking of going a bit further, a making it into a full survey with option fields and such.
However, I was going to do this once my shop goes on-line ....

The REAL IMPORTANT survey in my opinion is to know
Is your product arrived on time?
Is your product arrived in good conditions?
Are you satisfied with your purchase?
And other things to act like a indicator for factors i cannot measure with Google Analytics.
With G.A you can know where they came from.. i dont need to ask customers about this..
And I need the survey to be done days or a week after the purchase so i will give time for my customer to think rationally about the purchase..and not in the heat of the moment - the thrill of impulse buying.
