To see the files in file manager you have to un-hide protected files, then you will see .htaccess if it exists. Otherwise just rename htaccess.txt to .htaccess (yes the dot is first)

It should not, you have to use your domain.com/install to get it to install and you have to have created an empty mysql database on your host first.

You should take the upgrade path of from 1.1.1 to 1.5.6.4 then to 2.1.0.1 and stop there until v2.2.0.0 becomes production ready.
