That helper starts with if (extension_loaded('mbstring')) { which is probably false. That's why the utf8_ functions don't exist.
The webhost has to install it for you if you're on shared hosting

Undefined means it doesn't exist (even if you believe it does :)). Ask your host to install mbstring php extention or change utf8_substr to just substr and make sure your image filenames don't have 'strange' characters

In my humble opinion one should not use cache to hide server load and or coding issues. When cache expires you will have the same slow response. And what about ajax and other dynamic calls? eg add to cart, payment provider callback...

When subscriptions were added to oc only Paypal express was supported. I guess the oc guys thought a paypal invoice would suffice. You do get a transaction update though via the ipn. Permissions also rely on the usergroup. I suspect your server doesn't have apache suexec module installed which is probably why default user 'nobody' is the owner in stead of the account-user. This might be it but it's just a hunch, since you're on dedicated/unmanaged :)

It seems the rating is based on some kind of calculation. ie 2 five star shows as a 3-star rating, 4 five star as 4-star rating.
It looks like buyers didn't fully appreciate the extension which is incorrect. I'd say add breakdown like: 1 buyer(s) rated 5 stars, etc if you have to go this route.

You can try to find the 'missing' categories by using the wildcard % in the search box.
eg '%car' would also find 'ford 4wd cars'
Without the wildcard you'd have to type 'ford 4wd cars' in order to find it.

I suggest to open the browser developer tools and check for any 404 - not found assets, namely a css file.
It might also be useful to search the ocmod log for any NOT FOUND entries.
