Why would you want to have all 3 working? The normal course of action is to have your store setup as HTTPS and then use htaccess to have http redirect to https (and non-www. redirect to www.) On re-reading this thread is all you really want to do is;
strip .html if it exists? If there are some static HTML pages you wanted to exclude, they could be listed.

Ernie, the real issue is having 67,000 products and 4,000 categories, the bottleneck is pretty much the database. Although the database should scale up well and cache results automatically (in the engine) for so many products this is not feasible and increasing cache sizes etc. is not a solution as ...

This has been covered so many times, it would be helpful if you posted your url and opencart version (2.2 has a bug which needs additional fix) for starters.
Look at my posts half my replies relate to SSL issues and they are all resolved eventually.
