EDIT: Nevermind. I thought that the "Order Total" field was for what the handling fee should be called. Turns out it's a number- any order amount less than that gets charged the handling fee.

How do I turn on a handling fee?

I'm using OpenCart 1.5.1.3. I went into the Order Totals section in the admin panel, and enabled the "Handling Fee" option, set the sort order, and set an amount of "7.00".

When I visit the card, the handling charge doesn't show up.

How do I make it show up in the cart?

We have several users using it successfully right now. We have a few more tweaks to make before we call it "done" (at least with an initial feature set), but it should work well for 99% of users.
