Did you ever figure this out? I have having the same issue with some customers not able to view the form for the credit card info. They say it's just a line and not a form.
I agree it has to be something with their computer like an ad block or something

I just found this page and I had the same issue..it was caused by mod_evasive to control ddos attacks ... for me I had to do the follow to raise the limit of from Terminal I had to change DOSPageCount from 4 to 30 ... vi /etc/apache2/conf.d/300-mod_evasive.conf DOSPageCount 30 oh yeah and dont forge...

GREAT NEWS! I figured out the issue....on the new server BC Math isn't installed so I had to use Apache to rebuild the configuration to add BC Math to my php version!!! Boy it helps to log errors (system -> settings -> server -->>> log errors yes then and view from ftp on public html...just in case ...

The main error I keep seeing is....
There was an error when trying to sync locations and token: Domain name `www.mydomain.com` did not DNS resolvesquareup_error_field
Any ideas?
