Figured out the issue is with the Ajax Quick Checkout module I installed yesterday. Regular checkout works fine. So, marking as solved and will deal with the checkout author.

It seems like all the instructions to upgrade are - fresh install, rebuild, and move data. That just seems like a lot of work for a simple upgrade (from 3.0.3.2 to 3.0.4.0, or in my case, 3.0.3.8 to 3.0.4.0). Why is that the upgrade method? Is there any reason why you won't ask the extension author to fix this? Unless you got multiple store_ids, the product_id_store_id should be unique for a given product_id. I may do that but there are more errors cropping up and they all stem from the upgrade. Everything was working fine until then....

Upgrading to 3.0.3.9 I received the same error. Since it wasn't functionality I used, I just disabled Map on the Dashboard extension, which resolved it.
