With this fix, my system is running fast (sub-second response times) with 4124 categories in 3 layers.

The problem is not in the categories box, it is in the Search category drop-down in header.tpl. Check out the order checkout at mcmaster.com. They have a simple one-page layout designed to accomodate the "nuts and bolts" crowd -- which is basically the lowest common denominator in a customer base you can encounter.
