Maybe "Abandoned Carts"? And maybe they should be separate in the DB? Right now order ID is used to identify both the cart and then the order, which leads to the missing IDs. IMO it seems odd to create an order ID when there's no actual order. I understand the order ID has to be passed to the paymen...

If you skip step 3 you'd need to add a require call inside the send function

EDIT: sorry read skip not change, yes, you'd actually leave the mail require in the startup and just add a require for library/class.phpmailer.php

templates not showing in admin

on 1.3.0, added 2 templates in the catalog/view/theme but when i go to admin, default is the only available option... what did i miss? these templates were working on 1.2.9, did a fresh install and now it doesn't work
