ICON not showing on IE.

yup. i have seen it (the rates box) where i choose flat rates and inserted "2.00" for $2.00.

am i doing it right?

You never need to duplicate the flat shipping method. You just need to use the weight-based shipping method, set up a duplicate geo zone, and then create a high cost for each geo zone like: 99999:5.00 Or, you could use my Multi Flat Rate Shipping extension.
