I haven't given much thought to blocking IPs with this intentional way of causing errors – blocking them in an automatic way. I did an IP ban to those who tried to make an admin login attempt... the endpoint is fixed, so that was easy in some way. There was a period early this yr when > 100k failed ...

No, they aren't DOS. The server is UNIX, FreeBSD - 12.1.
I think you are right in your earlier reply, something more basic.
I haven't figured it, but UTF-8 encodings are not installed by default in FreeBSD, and that's what causing the problem.

Under pressure to roll-back due to a new-design launch starting today. I had it running live since 1st May, but I didn't catch the error. It is still there, under test-www.example.com instead. I'm not sure, I don't know where to begin... not the sharpest tool in the drawer :-)

Looks like instead of integer for country_id you're passing an object. I'm able to change the customer's Region/State internally from the admin side. Customers on the front-end cannot add a new address or amend an existing address. Changing the 'query' to below did not help either: $query = $this->...
