I'm using 1.4.9.5, after modifying the mod in order to make it compatible with 1.4.9.5 I was getting the same error message "Undefined variable: deliverydate_status in ..." ! so I'd to play around with it a little bit ... here is the link to the modified version, that I could make it work ...
