This was driving me nuts too. The solution that worked for me: disabling the "Rewrite Base" line in the htaccess file. That's it!

Thank you SO much, scanreg. I was banging my head against the wall trying to get the SEO URls to work. I couldn't figure it out, and no one at my server could figure it out either. Funny how the most frustrating problems have the simplest answer.
