If you are getting a 500 error then you will need to see the server error log. The opencart log will not show anything.

There is a small issue that the success page clears the cart and some session variables. So if the customer does not get to he success page then this will not happen. You might need to be a bit more specific.
If it is just an image upload then there is a file upload product option, but if you want to have a design interface for the customer then google "opencart product designer"

Possibly because your server is running unix/linux which is case sensitive.

If running Appache for the webserver, could try adding the mime type to the .htaccess and see if that works.

That should work in theory, but I suspect that it might be a little slow.

The payment gateway, do they pay directly on your site or do they get taken to the banks website to make the payment. I would have though that commbank would have a solution for you such...
