I am having problems with missing orders in OC 3.0.2; all cod and bank transfer payments go to missing orders, but order was placed succesfully.
I have read some possible solutions but doesnt work for me, like delete manually al status orders and place new.
I put a fresh install of Oc 3.0.3.0b in other server to see if error if fixed but error still persist.
What can I do?

Hello, I am stuck with this issue. I tested in a 3.1.0.0b fresh install and I can do first payment by Cod or bank transfer, but next orders shows success message in checkout page but in admin is a missing order.
Server error log not show any error.

In my case the culprit turned out to be browser caching rules I had set in htaccess (see attached). Maybe some AJAX calls were not made due to this. But when I commented out the three highlighted 3 lines (not sure which one is responsible, I suppose the first one), the orders started coming through normally.
Without it all orders missed their first line of order history set.
Now there is something else that I don't understand, though - the orders all come as "Processing", instead of "Pending" (at some point I had changed that setting in the OC settings, to see if that would help, but then I returned it to "Pending").
PS: For some reason my message got filtered as spam when I tried including the htaccess code in it, so I had to resort to putting it in an image - sorry for that.

It could be the ExpiresDefault line. There is a bug that was introduced in OpenCart 3 where no cache control headers are set. Setting an future expires date will most likely make the issue more noticeable.
