The first column is named Query.
When adding a new position, how to find out what to write in Query?

The query is basically the parameters of the URL for the page you're trying to rewrite. For example, if you want to rewrite an information page with an information_id of 4 to have a SEO keyword of "about-us", then you'd enter it like this:

Query: information_id=4
Keyword: about-us
Store: (whatever store you want it to apply to)
Language: (whatever language you want it to apply to)

You can also always do that when editing the product/category/manufacturer/information page, in the relevant area of the admin panel. The Design >SEO URLs area is just a way to add them more quickly.

Note that OpenCart can only rewrite products, categories, manufacturers, and information pages, so other things cannot be rewritten in the core functionality. If you do want to rewrite other URLs (like "route=account/login" or "route=checkout/checkout") then I have a Rewrite Manager extension that can do that.
