This is a strange case. I cannot reproduce this scenario but I have received order with same product in 2 lines.
This is a big problem because the stock is check against 2 separate lines.
The store only has 6 qty but the customer place 2 lines of this item.
Is it a bug? How can it be fixed.

Are you absolutely sure that the product IDs on those two lines are also identical or rather showing a different product ID of the same product in the cart?

I'm absolutely sure the 2 lines are with same product_id. We are selling wine and each wine is with unique product_id.
I even use phpMyAdmin to check the oc_order_product table. 2 lines are with same order_id & product_id.
All 3 times, the customers are placing order with our stock on hand quantity to 2 separate lines.
This make us very trouble to get the outstanding quantity.

Switch back to the default theme and see if this issue can be replicated.

Actually, I tried many times but can't replicate this issue but our customer can and seems to be intentionally break the order into 2 lines to place the order. They really want 2 times our stock quantity that cause problem.

I'll try with default theme.

No problem. Also ensure to clear your OC admin cache from the dashboard and from your admin - > extensions - > modifications - > refresh button noticing the changes in the cart page once you refill the identical products from scratch in the cart.
