Multiple Pick-up from Stores Locations, 2 levels

Hi, I need someone to customize a ocmod for me, to add a shipping method that based on the 'Pick-up from Store' concept.

I have 90+ stores in different locations, so showing them all in a Selection drop down or Checkbox is not going to look good.

So, I want to make it have 2 filters.
This extension is very close to what I want but there's only 1 level - https://www.opencart.com/index.php?rout ... n_id=19314

Like, first selection is the Geo Zone (Eg. New York), then inside New York he can select again from 10 of my New York outlets.
So instead of 1 selection box with 90+ locations, I can at least filter to 2 levels.
