Another Issue - State-Region reverting to no setting, Affecting Orders

Another issue I have been having is if a customer places an order it goes through fine but then say they come back at a later date they cannot place an order or have issues...

I traced this back to the "Region/State" setting defaulted to non set, so the order would not go through... I had to manually go through and update the "State/County" this is still happening at random... some stay set, others revert back to no State/County months later??

Any suggestions for these weird occurrences ??
