SEF URLs not coming, even after settings and htaccess is correct ?

Hello, we are on latest opencart and SEF is on in the backend.
htaccess is perfect.
mod re write is loaded too.

But still front-end SEF URLs are not coming.
Whats the reason ?

CAN SOMEONE HELP ?

My site loads fine with /index.php and admin same admin/index.php

But If I load direct URL for frontend or backend, it doesnt work
